To expedite the process, please submit the following details on an Incident/Claim Report Form:

  • Your firm’s contact information (name, address, phone number)
  • Your policy number
  • Date, time, and location of the incident
  • Brief description of the allegations
  • Names and contact information of those making the claim
  • Amount of any demand (if known) and date received

It is important to remember:

  • Do not admit liability or attempt to place blame
  • Accept any papers (documents, letters, memos, etc.) without comment or argument
  • Prepare a written, detailed account of the incident from your perspective
  • Collect and retain relevant documents for future reference, including:
    • Any lawsuit papers or legal proceedings
    • Relevant client contracts or agreements
    • Any other pertinent correspondence or documents
  • Make a copy of all records and correspondence for your files
  • Consult with your claims representative or assigned defense counsel prior to agreeing to participate in any discussions about the claim
